Annotation
OVERVIEW
In this section, a total of 88 incidents that occurred from 1959 through 1978 reflect experiences in refining and chemical unit operations. More than one-half of the incidents involve an explosion or fire, or both, while nearly another 25 percent occurred in periods of startup, shutdown, and turnaround.
